Mandatory OHSAA Physical Exam Forms
The OHSAA physical form must be completed and signed by a parent / guardian before the physical is given. All four pages must completed and signed in order for the physical to be valid and to allow the student to participate on an Athletic Team at New Albany. The physical form, as well as the other forms necessary for a student to play sports, can be found on this page.  All you have to do is click on all forms to print them, complete them, and turn them in.

Completed forms must be turned in to the Athletic Office before you can participate in any sport.  You must also go to the athletic office and pick up the OHSAA Athletic Eligibility Bulletin.  You are required to read this bulletin.  This form does not need to be returned.  Your signatures on the Athletic Participation Form acknowledges you read it.

1. OHSAA Pre Participation Physical Exam Form:
A doctor’s physical examination is necessary to verify that your athlete is physically able to participate in sports. If you are aware of special medical concerns of your athlete, we ask that you make the coach and athletic trainer aware of the problem. These forms MUST be completed and turned in to the coach BEFORE your athlete is eligible to practice.  Please get these forms completed as soon as possible so that your Student Athletes are able to begin practice at the proper starting date.  If you have any questions, please contact the Athletic Office at 937.878.6305.

"Respect the Game" is not telling you to behave in a "sporting" manner, whatever that might be. It's not about telling you what to do at all.  "Respect the Game": is just a reminder of what you already know: high school athletics is important and valuable.

As such, it's worthy of respect.
